Best English test for nursing registration
IELTS Academic or OET for nurse registration? Both are accepted by the NMC, AHPRA, NMBI and NCNZ, and OET grade B equals IELTS 7.0. Compare format and reach.
For nurse registration, both IELTS Academic and OET are accepted as equals by the NMC, AHPRA, NMBI and Nursing Council of New Zealand, with grade B equated to IELTS 7.0. OET tests healthcare English only; IELTS Academic also opens university study and many visa routes. Choose on task format and where else you need the result.
If you trained as a nurse abroad, the main regulators accept two English tests for registration: IELTS Academic and the healthcare-specific Occupational English Test (OET). They set the bar skill by skill rather than on an average, and treat OET grade B as the equal of IELTS band 7.0. The right choice depends less on which is "easier" and more on the task format that suits you — and on whether you also need the result for study or a visa.
At a glance
Both tests are marked to the same registration standard; what differs is the material, the task format and how far the result travels beyond the register.
| Dimension | IELTS Academic | OET |
|---|---|---|
| What it tests | General academic English, any field | Healthcare English only, profession-specific |
| Scoring scale | Nine-band scale (0-9), reported per skill plus an overall band | Grades A-E and a 0-500 score per sub-test; no overall grade |
| Writing task | Task 1 (describe a graph, table or process) plus a Task 2 essay, 60 minutes | One profession-specific letter, usually a referral, built from case notes, 45 minutes |
| Speaking format | Three-part interview with an examiner on general topics (about 11-14 minutes) | Two role-plays with an interlocutor playing a patient or carer (about 20 minutes) |
| Listening & Reading | General academic sources | Common to all professions, with a healthcare flavour |
| Accepted for nurse registration | Yes — NMC, AHPRA, NMBI and NCNZ | Yes — the same boards treat it as equal to IELTS Academic |
| Accepted beyond registration | University admission and many visa routes | Healthcare registration only (plus some linked health-worker visas) — not university or general migration |
| Regulator equivalence | Band 7.0 is the usual per-skill floor | Grade B equated to 7.0; grade C+ to 6.5 |
| Results speed | Computer-delivered in a few days; paper in about two weeks | Usually about two to three weeks — check the current timeline |
| Combining two sittings | Allowed by regulators under set conditions | The same concession where the board offers it |
Structural facts common to both tests; per-regulator scores, validity windows and results timelines are set by the test owners and each nursing board and reviewed periodically — confirm the live details on the official pages before booking.
Which should you choose?
Choose IELTS Academic if…
- You also need English for university study or a general visa route — OET is accepted for neither.
- You want the widest choice of test dates and centres and the deepest pool of free preparation material.
- You are comfortable with academic tasks — describing a graph and writing a discursive essay.
- You may move later on a skilled-migration route and want one result that travels beyond the register.
Choose OET if…
- You only need English for healthcare registration, and your board accepts OET.
- Writing a referral letter and role-playing a patient consultation suit you better than a graph and an essay.
- Clinical vocabulary is your strength while general academic topics are not.
- You want tasks set in the wards and charts you already work with — the pass standard is the same as IELTS band-for-grade.
How the nursing boards read the two tests
Every major English-speaking regulator — the UK's Nursing and Midwifery Council (NMC), the Nursing and Midwifery Board of Australia through AHPRA, Ireland's NMBI and the Nursing Council of New Zealand (NCNZ) — accepts IELTS Academic and OET as interchangeable proof of English for registration. Each judges the result one skill at a time, so your four component scores must each clear a floor and a strong overall figure cannot rescue a single weak paper. On IELTS that floor is band 7.0 in each skill, with some boards allowing a 6.5 concession in Writing; on OET it is grade B in each sub-test, with grade C+ standing in where the Writing concession applies.
The equivalence the boards work to is fixed and worth memorising: OET grade B is treated as IELTS 7.0, and OET grade C+ as IELTS 6.5. That is what lets a nurse meet the same standard on either test. The exact per-skill figures, validity windows and any two-sitting concessions are each regulator's to set and to revise, so treat the shape described here as settled and confirm the live numbers on the official regulator pages before you book.
What actually differs — the task format
Neither test is objectively easier; they ask for different things. OET sets every task inside clinical work: the Writing paper is a profession-specific letter — usually a referral — built from a set of case notes, and Speaking is two role-plays in which an interlocutor plays a patient or carer while you take the professional's part. IELTS Academic is general: Writing pairs a data-description task (a graph, table or process) with a discursive essay, and Speaking is a three-part interview with an examiner on everyday and abstract topics.
The Listening and Reading papers differ too. OET's are common to all twelve professions and carry a healthcare flavour, while IELTS draws on general academic sources. Which suits you is a matter of strengths: clinicians who find academic essay-writing awkward often prefer OET's letter, whereas candidates comfortable with general academic English — or who want the deepest pool of free practice material — tend towards IELTS. Both are marked to the same registration standard, so the decision is about fit, not about clearing a lower bar.
Where OET stops being an option
OET's specialism is also its limit. It is designed and accepted only for healthcare registration — and, in some countries, for the health-worker visa routes tied to that registration. It is not a university admissions test, and it is not on the general list of tests most immigration systems accept for study or skilled-migration visas. IELTS Academic, by contrast, is accepted for professional registration, university admission and many visa routes at once.
That difference matters if your plans reach past the register. A nurse who may go on to postgraduate study, or move later on a general skilled-migration route, gets more mileage from a single IELTS Academic result than from an OET grade that only the health regulators read. Where a visa is involved the rules vary by country and route — the UK, for instance, keeps its own UKVI-approved test list — so confirm what your specific immigration route accepts with the official source rather than assuming one test covers everything.
OET grades and their IELTS equivalents
For registration the nursing boards read the two tests through this equivalence. The IELTS bands are approximate; the pairings that matter are grade B = 7.0 and grade C+ = 6.5, which the regulators state directly.
| OET grade | OET numeric score | Approx. IELTS band | For nurse registration |
|---|---|---|---|
| A | 450-500 | 7.5 and above | Comfortably above the pass |
| B | 350-440 | about 7.0 | The standard registration pass |
| C+ | 300-340 | about 6.5 | Meets the Writing concession only (NMC, NMBI) |
| C | 200-290 | about 5.5-6.0 | Below the registration floor |
OET reports a grade and a numeric score per sub-test and no overall grade; the numeric boundaries are set by OET. The IELTS pairing is approximate except at grades B and C+, which regulators equate to 7.0 and 6.5 respectively. Confirm the current equivalence with your board.
What each nursing regulator asks
The per-skill shape is similar across the boards, but the Writing concession is not universal. This is the durable structure; every figure is the regulator's to set and revise, so confirm before booking.
| Regulator | IELTS Academic (per skill) | OET | Note |
|---|---|---|---|
| UK — NMC | 7.0 in Listening, Reading, Speaking; 6.5 Writing; 7.0 overall | Grade B (C+ Writing) | Two sittings combinable within six months |
| Australia — NMBA (via AHPRA) | 7.0 in every skill | Grade B | PTE Academic and TOEFL iBT also approved |
| Ireland — NMBI | 7.0 in Listening, Reading, Speaking; 6.5 Writing; 7.0 overall | Grade B (C+ Writing) | Combine-sittings concession |
| New Zealand — NCNZ | 7.0 in every skill (no Writing concession) | Grade B | Two dates combinable under NCNZ conditions |
Each board judges IELTS Academic and OET one skill at a time, never on an average, and only Academic IELTS counts — General Training is not accepted. See our IELTS for UK NMC nurse registration page for the full per-skill breakdown, and confirm live figures on each regulator's official pages.
Common questions
Is OET easier than IELTS for nurses?
Neither is officially easier. The nursing boards set OET grade B equal to IELTS band 7.0 and mark both one skill at a time. OET's tasks sit inside clinical work — a referral letter, a patient role-play — which many nurses find closer to their working English, while IELTS uses general academic tasks. Pick the format that plays to your strengths; the standard is the same.
Is OET accepted for UK nurse registration with the NMC?
Yes. The NMC accepts OET at grade B in Listening, Reading and Speaking, with grade C+ in Writing matching the IELTS 6.5 concession — fully on par with IELTS Academic. The two are treated as interchangeable for registration, so choose whichever suits you. See our IELTS for UK NMC nurse registration page for the full per-skill table.
Are PTE Academic or the Duolingo English Test accepted for nurse registration?
It depends on the regulator, so confirm before booking. The UK's NMC and Ireland's NMBI accept only IELTS Academic and OET. Australia's AHPRA additionally approves PTE Academic and TOEFL iBT. The Duolingo English Test is not accepted for nurse registration by these boards, even where universities take it — acceptance is the regulator's to set.
Can I use OET for a university course or a general visa?
No. OET is built and accepted only for healthcare registration, and in some countries for the health-worker visa routes tied to that registration. Universities and most immigration systems do not accept it — they take IELTS Academic. If you need English for study or a general visa as well as the register, sit IELTS.
Does OET grade B equal IELTS 7.0?
For registration, yes. The nursing boards treat OET grade B as equivalent to IELTS band 7.0, and grade C+ as IELTS 6.5. That equivalence is exactly what lets a nurse meet the same standard on either test. The pairing at other grades is only approximate — confirm the current equivalence with your regulator.
Is IELTS General Training accepted for nurse registration?
No. Every major nursing regulator that takes IELTS requires the Academic module, not General Training. Listening and Speaking are identical across the two versions, but the Reading and Writing Task 1 papers differ, so prepare on Academic material from the start rather than the General Training equivalents.
