For an internationally educated nurse heading to the United States, the IELTS requirement is fixed: the VisaScreen certificate issued through CGFNS asks for IELTS Academic with an overall band of 6.5, Speaking of at least 7.0, and no individual band below 6.0 — all achieved in one sitting. General Training and at-home IELTS Online results are not accepted.

One more constraint matters for planning retakes: IELTS One Skill Retake is not available for tests taken in the USA as of 2026. If one band falls short, the whole test must be retaken at full price — which makes targeted preparation before the first attempt considerably cheaper than a strategy of quick retakes.
Why does VisaScreen matter so much?
The VisaScreen certificate is required by the US Department of State before an employer can file an occupational visa petition (such as EB-3) for a foreign healthcare worker. Separately, roughly two-thirds of US State Boards of Nursing require CGFNS certification before approving international candidates to sit the NCLEX-RN. The English test is therefore not a formality — it gates both the licence pathway and the visa pathway.
The rule is profession-specific: IELTS is accepted for registered nurses, but physical therapists and occupational therapists cannot use IELTS for VisaScreen — their certifying bodies require TOEFL instead. If you are not an RN, confirm which test your certifying body names before booking anything.
How should a nurse train for the Speaking 7.0?
Most nurses who miss the VisaScreen requirement miss it on Speaking. Band 7.0 Speaking rewards sustained, well-organised answers with flexible vocabulary — exactly what clinical professionals can produce about their own field, but rarely practise under timed conditions. Practise full 11-to-14-minute speaking sessions rather than isolated questions, and record yourself: hesitation patterns and short answers are what hold a fluent speaker at 6.5.
- Practise the full three-part Speaking format under time, including a Part 2 long turn every session.
- Because the single-sitting rule bars combining results, rehearse all four modules together in full mocks, not one module at a time.
