IELTS for US nurses: the CGFNS VisaScreen requirement
Every internationally educated nurse applying for a US occupational visa clears a CGFNS VisaScreen assessment, and English proficiency is one of its components. For IELTS that means the Academic paper on a two-tier model — an overall band plus a higher Speaking minimum — with your state Board of Nursing free to set its own English rule on top. Knowing which body checks what, and which module carries the higher bar, is where preparation starts.

Which IELTS does VisaScreen accept?
CGFNS runs the VisaScreen assessment that internationally educated nurses need for a US occupational visa, and its English-language component is met with IELTS Academic — not General Training. Academic is the paper CGFNS lists because its Reading and Writing sit in the register a clinical role uses; the Listening and Speaking papers are identical to those on General Training.
IELTS is not the only route: CGFNS also accepts TOEFL iBT and PTE Academic, each at its own passing score. Whichever you sit, CGFNS certifies against the score it publishes for your occupation, so confirm the current figures on the official CGFNS VisaScreen page before you book.

The two-number rule: overall 6.5, Speaking 7.0
The IELTS Academic standard for registered nurses is a two-tier one. You need an overall band of 6.5 and, separately, a Speaking band of 7.0. There is no extra per-module floor on Listening, Reading or Writing beyond what the overall band demands — Speaking is the single module held to its own higher minimum.
Speaking carries the higher bar for the reason the assessment exists: a nurse who cannot be understood at the bedside is a safety risk however the reading score falls. Every accepted test lifts the same thing — TOEFL iBT and PTE Academic each set a raised speaking sub-score too — so build your preparation around the module that most often blocks otherwise strong candidates.
Your result should come from a single valid test sitting within the currency window CGFNS accepts, rather than modules banked across different dates. Treat the whole paper as one target and aim to clear every band in one attempt.

VisaScreen is federal — your state board is separate
VisaScreen clears you for the visa. It does not license you to nurse. A US registered-nurse licence is granted by the Board of Nursing of the individual state you intend to work in, and each board sets its own eligibility rules — including whether it wants separate evidence of English, which tests it accepts, and at what score. Some boards lean on much the same exam picture as VisaScreen; some require none; several differ.
So a US-bound nurse can be clearing two English checks that look alike but are set by different bodies: the federal VisaScreen standard for the visa, and a state board's rule for the licence and NCLEX-RN eligibility. Confirm your destination state's Board of Nursing requirements alongside the CGFNS ones, because meeting one does not automatically satisfy the other.

When a nurse is exempt from the English test
CGFNS can waive the English component for nurses whose education was delivered and examined in English in a country on its recognised list — majority-English jurisdictions such as Australia, Canada, Ireland, New Zealand, the United Kingdom and the United States. The exemption turns on the language of your training and textbooks, not on your nationality.
If you trained in English but outside those recognised countries, or the position is unclear, you sit the test like everyone else. Eligibility for the exemption is decided by CGFNS, so check your case with them before assuming you can skip IELTS — and where there is any doubt, prepare as though you must sit it.

VisaScreen English standard — IELTS Academic (registered nurse)
The stable CGFNS scoring model: one module is held higher than the rest. Confirm the current passing scores for your occupation on the official CGFNS VisaScreen page.
| Skill | VisaScreen minimum | How it is set |
|---|---|---|
| Listening | Counts toward the overall band | No separate module floor |
| Reading | Counts toward the overall band | No separate module floor |
| Writing | Counts toward the overall band | No separate module floor |
| Speaking | 7.0 | The one module with its own higher minimum |
| Overall band | 6.5 | The score CGFNS certifies against |
This is the registered-nurse standard. CGFNS sets its own passing score for each accepted exam (IELTS Academic, TOEFL iBT, PTE Academic) and for each health occupation, and it accepts results within a currency window — vocational and practical-nurse routes and other professions can differ. Verify the figures that apply to you on the CGFNS website.
The two English layers a US-bound nurse clears
The visa and the licence are decided by different bodies, and each sets its own English rule — meeting one does not satisfy the other.
| Requirement layer | What it governs | Its English rule |
|---|---|---|
| CGFNS VisaScreen (federal) | The occupational-visa screen for healthcare workers | One accepted exam at CGFNS's passing score — for IELTS Academic, 6.5 overall with Speaking 7.0 |
| State Board of Nursing | Your licence and NCLEX-RN eligibility in that state | Sets its own rule — some boards accept an English exam pass, some require none, and accepted tests and scores vary by state |
| Employer or visa sponsor | The job offer behind the petition | May set a higher bar for the role, but cannot waive the VisaScreen floor |
US nursing practice and the NCLEX-RN are conducted in English, but the licence itself is granted by the state board, not by CGFNS. Confirm your intended state's Board of Nursing requirements and the current VisaScreen passing scores on the CGFNS website.

Frequently asked questions
Which IELTS do US nurses need for VisaScreen — Academic or General Training?
IELTS Academic. The CGFNS VisaScreen English standard for registered nurses is an overall band of 6.5 with a Speaking band of 7.0. General Training is not the paper CGFNS lists. Confirm the current scores for your occupation on the official CGFNS website.
Why is the Speaking requirement higher than the other modules?
Because the assessment exists to protect patient safety, and a nurse who cannot be understood at the bedside is a risk however the reading score falls. Speaking is held to 7.0 while the other three modules are governed by the 6.5 overall band. Every accepted test raises its speaking measure the same way.
Do I need separate minimum bands in Listening, Reading and Writing?
For the registered-nurse standard, no — there is no extra per-module floor beyond the 6.5 overall band, and only Speaking carries its own higher minimum of 7.0. CGFNS can set different scores for other occupations, so confirm the figures that apply to your route.
Is IELTS the only test VisaScreen accepts?
No. CGFNS also accepts TOEFL iBT and PTE Academic, each at its own published passing score, and each lifts its speaking sub-score in the same way IELTS lifts Speaking to 7.0. CGFNS publishes the exact numbers for each test and occupation.
Does passing VisaScreen mean I can work as a nurse in the US?
No. VisaScreen clears the occupational visa; it does not license you. Your US registered-nurse licence is granted by the state Board of Nursing, which sets NCLEX-RN eligibility and may have its own English rule. Requirements differ by state, so check your destination board as well as CGFNS.
Can I be exempt from the IELTS requirement?
Possibly. CGFNS can waive the English test for nurses educated and examined in English in a country on its recognised list, such as Australia, Canada, Ireland, New Zealand, the UK and the US. The exemption depends on the language of your training, not your nationality, and CGFNS decides eligibility — confirm your case with them.
